How to Complete a HP Webform for Power Cord Replacement

Determine if the Power Cord Needs Replacement

  1. Unplug your power cord from the wall outlet.
  2. Locate the molded mark on the adapter end of the power cord.
  3. If the mark reads, “LS-15”, you have a potentially affected power cord and K12 encourages you to complete the Recall Instructions below.

Note: If you do not have a LS-15 power cord, please continue using the cord that was sent to you.

Getting Started

Please go to the HP website: https:// h30652.www3.hp.com/ and then follow the instructions below. Entering the responses in the instructions will ensure that you are able to complete the process with ease.

  1. Fill out all the required fields in the Getting Started section:
    • Country
    • Email Address
  2. Select YES to: “Does the molded mark include LS-15?”

Validate Your AC Power Cord

In order to continue, you will need to know the notebook (laptop) Product name and serial number. To locate this information:

  1. Hold down the Function [Fn] key (lower left on keyboard) while pressing the Esc key (upper-left on keyboard).
  2. Your computer will display the information.

Note: If the step above does not display the information, check the product label located on the bottom of your unit or inside the Hp proBook 4520S laptop battery compartment.

  1. Enter the Notebook Product Name and Serial Number into the designated fields on the HP Recall page.
  2. Please select the following responses to the next set of questions under IDENTIFY AND PROVIDE THE DATE CODE FOR THE AC POWER CORD and ADAPTER CT CHECK (even if you can see the Date Code):
    • YES – Do you have one of the pictured labels as shown above?
    • NO – Can you read the numbers on the label?
    • NO – Do you have the HP Pavilion DV6000 AC Adapter that your power cord plugs into?
  1. Retype the “characters” in the validation picture and then click Validate.

Ship To Information and Plug Type

  1. Please complete all required address fields in the “Ship To Information” section and then click Place order.
  2. Since this is an International recall, you will be asked to verify the type of plug you need for the AC Power Cord. View the picture and then click Continue.
